LegalNotice of Privacy Practices
Confidentiality of participant records · Last updated: October 2026
This notice describes how information about the individuals we serve may be used and disclosed, and how you can get access to this information. Please review it carefully.
Simply Lovely Services LLC provides Coordinated Family Support to individuals with developmental disabilities. We are committed to protecting the confidentiality of the personal and health-related information (“records”) of the people we serve, and we handle those records with care and in accordance with applicable law.
Our commitment
We treat participant records as confidential. Access is limited to staff who need it to coordinate services, on a “minimum necessary” basis, and all access is logged.
Legal protections
Records of individuals who receive services through California's developmental services system are confidential and protected under California law, including Welfare and Institutions Code sections 4514 and 5328. Where applicable, we also apply HIPAA-aligned safeguards to protected health information.
How records may be used and disclosed
- For coordination of services — to plan, provide, and coordinate support with your regional center and care team.
- With your authorization — for any purpose you approve in writing. You may revoke an authorization at any time.
- As required or permitted by law — for example, mandated reporting of suspected abuse or neglect, responding to a valid legal order, or to prevent a serious threat to health or safety.
We do not sell participant information, and we do not use or disclose it for marketing without authorization.
How we safeguard records
- Encryption of records at rest and in transit;
- Role-based, minimum-necessary access controls;
- Mandatory two-factor authentication for all staff;
- Audit logging of who accessed or changed a record and when;
- Regular backups, monitoring, and staff confidentiality training.
Your rights regarding records
- Access — request to review or receive a copy of the individual's records (subject to legal limits);
- Correction — request a correction to information you believe is inaccurate;
- Accounting — ask about certain disclosures we have made;
- Restriction — request limits on certain uses or disclosures;
- Complaint — raise a concern with us without fear of retaliation.
Breach notification
If a breach involving unsecured records occurs, we will notify affected individuals (and regulators where required) in accordance with applicable law.
